What will happen at my first visit?
Please assist us at the time of your initial visit to the office by providing the following information:
- - Your referral slip and x-rays from your referring dentist.
- - A list of your medications and your physicians (please note that you can fill out your patient registration and medical history form on web page.)
- - If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to your first appointment (e.g. artificial joints, heart valves or heart murmurs requiring pre-medication with antibiotics.)
We will complete a medical history review, an in depth dental history evaluation and a thorough examination to discover signs of destructive periodontal disease including gum pocket formation, gum recession, loss of bone and loosening of teeth.
You will be screened to rule-out signs of oral cancer or other oral pathology. If you are a candidate for dental implants, additional records (study models, special X- Rays) may be needed.
